Appointment Policy
We operate on an appointment basis, but will treat an emergency immediately.

New Client Form
Please complete this form and either fax it to 480-287-8696 or or bring it with you to the hospital the day of your appointment. If you fax it, please include the date and time of your appointment in the subject line.
Pre-Surgical Policy
Prior to any surgery, a medical treatment plan outlining specifics o the
procedure is provided and reviewed with each client. Pre-anesthetic
blood work may be required. In addition, we ask that your pet is not fed
after midnight preceding the day of blood work and/or surgery.
Overnight Patients
For overnight patients, a staff member lives on the premises and
monitors animals throughout the night.
Payment Options
BHPH accepts cash, personal checks, Visa, MasterCard, American Express,
Care Credit and traveler's checks. Payment is required upon completion
of services. A deposit may also be required for in-hospital critical
care.
